Living with Partial androgen insensitivity syndrome

A condition in which the body only partly responds to male sex hormones, leading to a range of differences in genital development.

Understanding Partial androgen insensitivity syndrome

Partial androgen insensitivity syndrome (PAIS) is a disorder of sex development in which the body partly responds to male sex hormones (androgens).

It affects the reproductive and genital organs as a baby grows before birth. It happens in people with a 46,XY karyotype (the usual male chromosome pattern). Signs vary widely, causing a range of differences in how the genitals look. In milder cases, infertility may be the only sign.

Signs & symptoms

Symptoms vary from person to person. GARD lists these among the most common:

  • Differences in how the genitals look (ambiguous genitalia)
  • Opening of the urethra on the underside of the penis (hypospadias)
  • Testes that haven't moved down (cryptorchidism)
  • Enlarged breast tissue in males (gynecomastia)
  • Infertility
  • Higher testosterone and other hormone levels

Not everyone has every symptom. GARD's list comes from the Human Phenotype Ontology.

Diagnosis

  1. Talk with your doctor

Share the symptoms you've noticed, when they started and your family health history. Notes and photos can help.

  2. See a specialist

Specialists who may be involved include genetic specialists, hormone specialists, maternal-fetal medicine specialists and urinary and male reproductive system specialists.

  3. Consider genetic testing

Changes in AR are linked to this condition. Genetic testing and counseling can help confirm a diagnosis and explain what it means for your family.

  4. Connect with expert centers

Resolve: The National Infertility Association may keep lists of experts for this condition.
